The General Information & Inquiries form can be submitted by mail, fax or online, or a combination.  The other forms need to be printed and either scanned or faxed when submitting them.  If you have selected Captain Only Charter the 'provisioning requests', & 'scuba' should be submitted by each individual guest but the 'general information', 'activities' forms should be filled out by one person summarizing the group's preferences. 

• The arrival information and guest list can be completed by anyone as information becomes available.
• If there are divers in the party they should each complete the scuba diving requirements form (please try to put everyone's information on one form).
• Provisioning requests are built on a concensus of our guests
• Guests should summarize the group's tastes on one form,  listing any specific likes, dislikes and more importanly food allergies.
• Please give as many detail as possible - so, the crew can plan the menu or activities that fit your preferences.
• Once you have filled out the general information & inquiries form, we will send you our brochure and charter preference forms: Activities & interest, Provisioning request, arrival information, Scuba diving.
• Be sure to include any medical information which will help us provide a safe and enjoyable cruise.
